Make a Difference in Patient Care: We are seeking a dedicated and compassionate Medical Officer to join our Home-Based Paediatric Palliative Care Team. This role involves providing medical care and symptom management to children with life-limiting or life-threatening conditions in their homes.
How You'll Contribute:
- Provide medical assessment, diagnosis, and symptom management for paediatric palliative care patients in home settings.
- Develop and regularly review individualized care plans in collaboration with the multidisciplinary team.
- Offer emotional support and clear communication to families during complex and sensitive situations, including end-of-life care.
- Participate in home visits, OPD at South Goa district hospital with the care team, ensuring continuity of care and crisis management when needed.
- Collaborate with community providers, hospitals, and specialists to ensure coordinated care.
- Maintain accurate and timely medical records, reports, and documentation.
- Attend weekly meetings, training programs and activities planned for children and their families.
